On and Off Exchange Health Plans: Compare Both Before You Choose

Some individual health plans are sold through the HealthCare.gov Marketplace and some are sold directly by insurance companies. HealthWon Networks compares both, so you can see which route gives you the best mix of price, network and savings for your household.

What On-Exchange and Off-Exchange Really Mean

The "exchange" is the Health Insurance Marketplace, which in Louisiana, Texas, Mississippi and Missouri is HealthCare.gov. An on-exchange plan is one you buy through the Marketplace. An off-exchange plan is individual or family coverage you buy outside the Marketplace, directly from an insurance company or through a licensed agent. Both kinds of ACA-compliant major medical plans cover the same essential health benefits and can't deny you for a pre-existing condition.

The big difference is money. Premium tax credits and cost-sharing reductions are only available on-exchange. Off-exchange plans can still make sense when you don't qualify for savings, or when a plan or network you want isn't offered on the Marketplace. HealthWon Networks has more than 15 years of insurance experience, and our agents are licensed in Louisiana, Texas, Mississippi and Missouri. We lay both options next to each other so you aren't guessing.

On-Exchange Plans Through HealthCare.gov

Buying on-exchange is the only way to use a premium tax credit to lower your monthly premium, and the only way to get cost-sharing reductions on a Silver plan. The Marketplace application also screens your household for Medicaid and CHIP. If your income makes you eligible for savings, on-exchange is usually where you'll find the lowest net premium, and we help you fill out the application and choose a plan.

Off-Exchange Plans Sold Directly by Insurers

Off-exchange coverage can be a good fit if your household income is too high to qualify for a premium tax credit, or if an insurer offers a plan design or network outside the Marketplace that suits you better. ACA-compliant off-exchange plans follow the same consumer protections as Marketplace plans, but you pay the full premium with no tax credit. We compare them against on-exchange options using your real numbers, not just the sticker price.

How We Decide Which Route Fits

First, we estimate your household income and check whether you'd qualify for a premium tax credit or cost-sharing reductions. Then we compare similar plans on and off the exchange, check provider networks for your doctors and hospitals, and look up your prescriptions on each plan's drug list. Finally, we compare total yearly cost, meaning premiums plus the deductible and out-of-pocket maximum, so you can see what each choice could really cost in a year when you need care.

Short-Term and Supplemental Coverage

Short-term health plans and supplemental plans are not ACA-compliant major medical coverage. They can exclude pre-existing conditions, may limit what they pay, and don't qualify for premium tax credits. Some people use them to fill a specific gap or alongside a major medical plan. If you're considering one, we'll explain clearly what it does and doesn't cover so you can decide with the full picture.

Enrollment Dates for 2027 Coverage

ACA-compliant individual plans, on or off the exchange, generally follow the same enrollment window. In Louisiana, Texas, Mississippi and Missouri the HealthCare.gov schedule for 2027 coverage is:

  • November 1, 2026Open Enrollment starts for 2027 coverage.
  • December 15, 2026Last day to choose a plan for coverage that starts January 1, 2027.
  • January 15, 2027Open Enrollment ends. Plans chosen December 16 through January 15 generally start February 1, 2027.

On and Off Exchange Health Plans FAQs

Can I use a premium tax credit on an off-exchange plan?

No. Premium tax credits and cost-sharing reductions are only available on plans bought through the Marketplace, which is HealthCare.gov in Louisiana, Texas, Mississippi and Missouri.

Do off-exchange plans cover pre-existing conditions?

ACA-compliant off-exchange major medical plans do. Short-term and many supplemental plans don't have to, which is why we explain the difference before you buy.

Can I buy an off-exchange plan at any time of year?

ACA-compliant individual plans, on or off the exchange, are generally sold during Open Enrollment or a Special Enrollment Period. For 2027 coverage, Open Enrollment runs November 1, 2026 through January 15, 2027.

Which is cheaper, on-exchange or off-exchange?

It depends on your income and the plans available in your county. If you qualify for a premium tax credit, on-exchange usually costs less. If you don't, we compare similar plans both ways using your doctors, prescriptions and budget.
